| Please enter all requested information needed for registration. When you drop off your children at our Jacksonville location for the first time, you will be asked to sign our policies and guidelines and your registration will be completed at that time.
**IMPORTANT** YOU MUST BE THE PARENT OR LEGAL GUARDIAN OF THE CHILD TO COMPLETE THE REGISTRATION AND SIGN THE PAPERWORK. You are not legally permited to register a child that is your Grandchild, Niece or Nephew, Neighbor or any other situation unless you have proper documentation showing that you are the LEGAL GUARDIAN of that child.
